Step 1: Intake Form and Waivers
Please fill out the intake form you receive via email before your acupuncture or herbal appointment! If you prefer, you can also fill out paper forms ahead of time, or I will have paper intake forms in clinic. Just arrive 20-30 minutes prior to your first appointment if you would like to fill them out in clinic.

Step 2: Review Appointment Expectations and Space
Let's acknowledge the COVID-sized elephant in the room.

- Masks required indoors if you are feeling sick, have allergy cold or flu symptoms. Masking even if you are usually healthy helps keep the space accessible for those who may have autoimmune conditions or are otherwise at high risk for hospitalization with COVID-19.
- If you were in close contact with a COVID-19 positive individual in the last 5 days, or testing positive, please reschedule. There will be no penalty for rescheduling due to COVID-19 related concerns. I follow CDC guidelines for the clinic's quarantining policies.
- We sanitize the heck out of surfaces after each patient.
- Cell phones must be off in the clinic. You’re welcome to listen to music on your device with ear buds, but just make sure your phone is not making sounds or vibrating, or that you are not loudly fighting with your Office Enemy, especially in the reception area.
- Please enter and exit the treatment room quietly and speak softly when talking in the reception. Acupuncture is usually very relaxing, and people often fall asleep. We also share space with a massage therapist who does her best to provide a quiet space for her folks too.
- Sometimes we have sweet senior dogs in the waiting room the chiropractor is seeing. You may hear their click clacking of their nails on the hardwood occasionally.
- While a treatment usually lasts about 30-60 minutes, please let us know if you are ready to go or if you want to be out by a certain time.
- Eat a little something before your treatment. Acupuncture is not recommended on an empty stomach.
- If possible, wear loose, comfortable clothing so we can access just above your elbows and knees easily. If points are not accessible, we will ask you to undress to your comfort level and drape yourself with sheets, much like if you were receiving a massage.
- Please let us know if there are any areas of your body that you would like us to avoid treating, touching or acknowledging. We want to make this as safe of a space as possible for you.
- Please arrive on time. If you arrive more than 15 minutes late, we may not be able to treat you depending on the schedule, and we just won't have as much time to delve deeply into what is going on.
